Your First Visit

Before your first appointment, we welcome you to peruse our website to learn more about the services and treatments we offer and what sets our dental practice apart from others. We also encourage you to read through this page to learn more about how to prepare and what you can expect at your first appointment with Dr. Charise Petrelli or Dr. Cathy Nguyen. 

What To Bring to Your First Appointment

To help our office staff complete your patient file, we ask that you bring the following documents and information to your first appointment:

• Identification
• Your dental health insurance identification card or policy
• New patient forms (available on our website or in our office to complete at the time of your appointment)
• Medical history
  • A list of current medications and supplements in addition to information regarding health conditions, devices, or medical history that might affect your dental care, such as:
    • High blood pressure (hypertension)
    • Artificial heart valves
    • Pacemaker
    • Diabetes
    • Lupus or other immune system disorders
    • Artificial joints
    • Rheumatic fever
    • Other medical conditions
• Prior dental records and dental work (Please request a copy or ask your previous dental provider to forward these to our office.)
• Dental X-rays or other diagnostic images (from your previous dentist or current physician)

If you plan to fill out your new patient forms in our office at the time of your appointment, we recommend arriving for your appointment a few minutes earlier to ensure you have ample time to complete this paperwork. 

Additionally, all patients who are under the age of 18 should be accompanied by a legal parent or guardian. 

What To Expect During Your First Visit

During your first visit, we take special care to ensure you become thoroughly acquainted with our office and dental practice. We want you to feel comfortable with the care you are receiving and have all your questions answered. 

During your first appointment, our dentists work to establish a solid baseline of your dental and general health, getting acquainted with your medical and dental records and examining your teeth and gums. We also talk with you about any past or present concerns you might have with your oral health and your current oral health goals so that we can set you up with an appropriate treatment plan designed to meet your dental care needs while helping you achieve your smile goals. 

Depending on why you are visiting Glen Ellyn Dentistry for the first time, the exact components of your appointment might vary. For example, appointments for a routine examination and cleaning differ from the first appointments needed for addressing specific concerns such as periodontal disease or cosmetic treatments.
